Food As Medicine
Welcome to Food as Medicine with The Food Pantries for the Capital District. Our Food as Medicine program connects those living with both a chronic health condition and nutrition insecurity with wholesome, nutritious, and balanced groceries and meals medically tailored for their health. This 6-month weekly program has two types of services:
Medically tailored groceries (MTG): A weekly package with enough groceries for at least 10 meals per person in the household. Appropriate for those individuals living with diabetes, heart disease, high blood pressure, and obesity.
Medically tailored meals (MTM): Delivery of three prepared meals for each day of the week per person in the household. Appropriate for those living with diabetes, heart failure, cancer, and HIV/AIDS.
Participation in the program requires a medical screening and referral by one’s primary care provider to determine eligibility. Please use the contact form below if you would like to learn more.
We wanted to provide you with access to recipes, tips, and information to help you with developing a healthier relationship with food. This information is yours to use forever and we hope you will continue on your healthy food journey with these resources long after you've completed the program.
